Teaching woes

A dilemma for all you teachers out there:

I have a student who is fairly bright, polite, etc etc, but I am having a problem with her in her lessons. Every other lesson, she is well prepared, has the material down, has obviously practiced and absorbed the concepts at hand...and then there are the other half of the lessons, where this is very obviously not the case. We have one great lesson where things get done and e move on, and the in the next lesson she is completely unprepared, and we end up not getting much done. I have even mentioned to her on the "off" weeks that "one of our goals for next week will be to practice more consistantly" - and she looks sheepish, but nothing changes. When she's on, she's on, but I don't quite know how to broach the subject correctly - in my experience, students are either one way or the other - not both alternately!
Has anyone else run into this? How did you handle it?
