Think of what Doug describes as the "break" that a clarinet player experiences when going from the lower to the upper registers.

The only way to get out of the habit is to play across it so many times that there is no break. Buzzing the mouthpiece, singing the pitchs, hearing the pitchs, and then using your air consistently are the only ways I have found to work. One replaces a bad habit with a good one.
