It would include any money that in any way go from a person to the local or national governments, including income tax, sales tax, import tax (where applicable) road tax, etc etc etc. If you investigated this in the US you might be surprised over the level, though I would be surprised if it came anywhere near 93%. The issue over taxation/income/survival (as you put it) would be better described in studies on buying power, where these differences between USA and Denmark would look very different from what comes from this discussion.
