Without grabbing a slide rule...uhhhh...CALCULATOR, yeah, that's it!...and using the "12th root of 2" formula, a real rough guess would be that

a couple of eight-inch-long "piggy-back" main tuning slide extensions (as long as this didn't make the main tuning slide drag on the chair) might do the trick, and the other slides should all have enough pull range to accommodate...

OR...

The Eb tuba's 5th valve slide could be built with enough "range" that it could be pushed in SHORT enough to be the same length as the "regular" #1 slide. Then, one could:

- push in the #5 slide all the way
- pull out the 1,2,3,4 slides to be in tune in the key of D

and have a 4-valve D tuba (holding down [or wedging down] the 5th valve spatula all the time)
