Yes 4&5 would be the correct fingering for low E. Works a lot nicer than the more typical 234 used by folks with a flat whole step fifth. I actually prefer the double whole step fifth valve but based on all the new horns that are only available with a flat whole step I'm in the minority.

8va technically just means that the composer wants that part in another octave. 8vb always means take it down an octave. When I see 8va I try to take the context of the music into consideration when deciding which direction to go.
