I still remember at four years old visiting an "old lady" across the alley who played the organ....and who counted out loud in a gravelly old lady voice. I was so embarrassed to even be HEARING it, I would NOT count out loud until I was an adult teaching students!!
This kid may simply be incredibly self-conscious due to similar embarrassments, and the trick might be to find an approach that circumvents that.
Or, if he is ADHD, there are nutritional deficiencies that contribute to that, almost always overlooked by the Pharmaceutical Cartel that is currently running the country. If you want to know more about that....a web search might turn up some really interesting info.
