I've got a Bach 18 less than 10 years old with a special-order small shank and I believe that Schilke still advertises that they'll take special orders for nonstandard shanks. Don't forget Doug Elliott, who offers a bewildering range of shanks. And I'll bet that if you ask Terry Warburton or Joe Marcinkiewicz (or most any of the small-shop mouthpiece maker) they'd be happy to make up a mouthpiece with a custom shank for an extra charge.
