Perhaps your jaw isn't open enough, or your tongue is too high in your mouth, or you're not inhaling/exhaling properly. Perhaps you need to practice buzzing long notes and slow etudes on the mouthpiece. Perhaps a combination of these things.
It's hard to tell without seeing you play, so refer to the first point.
