I own a mini-score of the VW concerto. The high ab WAS in the original. The publishers dedicate a whole page at the start of the book explaining the cadenza.

Phillip Catelinet, the late tubist of the LSO who premiered the work, told VW and the publisher that the high Ab was out of reach of most tubists of the day. Therefore, it was omitted from the part until a few years ago. A fellow called Roy Douglas, who worked extensively with VW and Catelinet on the work, was called by Oxford Press when they revised the work a while back. Amongst the numerous articulation and phrasing changes he made, he said that VW would've liked the high Ab's to be reinstated.....so they were.
