My mother has the same problem and she uses a mouthpiece quite a bit (though she's a violinist). I believe the problem stems from psychological stress. Perhaps you need a few days off? Also, if possible, you may wish to seek out a dentist who also plays a brass instrument. There's a bass-trombonist dentist where I live and he understands the delicacies of a musician's embouchure very well.
