You probably know plenty about all this but here's my take:

Dunno what you've been doing exacly but it sounds like you're trying to do too much too fast. When your chops are tired as you describe just take it easy. Don't try to force damaged equipment (your embouchure) to work at top rate without extending the damage. Stick to low intensity playing in the mid & mid-low register until your chops feel better. If you have rehearsals and gigs you can't avoid, just generally take it easy... It is possible you've over-stressed your embouchure with too much extended high reg playing? Once you're back practising properly with 100% lips, practice your upper reg sensibly and steadily.
