I think that this point was neglected. Try singing the pitches first. One you can sing them in tune with your tuner, then you can easily transfer those in tune pitches to your chops, thus causing them to not crack. People often overlook the importance of hearing pitches in one's head before playing them. As someone also mentioned... make sure that your horn's in tune too.
