This is what's known as an echo attachment, or echo bell. It is used in solistic playing to get a muted or distant sound, usually to "echo" the primary sound. Someone better versed in the terminology can better explain when it would be used musically. This works similarly to a double bell euphonium in that it has a valve (the shorter first one) that diverts the sound from the main bell to the "echo" bell. I have never seen one used, but the guy from whom I bought one of my double bell euphoniums had a Henry Distin echo bell cornet from the 1890s. I also have an old Distin catalog that shows an alto horn made with an echo attachment. I have never seen one of these in person.
