Sounds like a bit too much pressure btweeen mouthpiece/upper lip plus maybe a general tendency to overblow slightly all the time.
Try this and see what effect it has: To equalize pressure on your lips move botom-part of your horn towards you slightly / maybe have a repairman change the angle of your lead-pipe, so you get some pressure off the upper, onto your bottom-lip. Secondly, try to play with less power, less energy, more in a leggiero mood all the time. More of a "letting it happen", less "Making it happen" thingy..
