I recommend trying the SharpEye music reader which can be downloaded and tried for free from http://www.visiv.co.uk/about.htm. When I tried it, I used the following steps:

Scan a hymn from the hymnbook
Enhance the scan - eventually save to crisp black and white bmp file
(this is really the hardest part for me)
Load bmp into SharpEye, scan, save to midi file.
Import midi file into notation software of choice.
Fix errors, separate parts, transpose, and print.

It's cumbersome but if I want all parts, or to transpose a piano piece, it's the only way. It's not worth it for transposing a single part (which is what I mostly do for the church trumpet player) so I haven't done it in a while.
