Starting again will only really work if the desire is there. Otherwise, you will struggle with everything. Once the desire to play is acheived, take it easy. Playing the basics again, Kopprasch, Blazhevich, Rochut, Arban for fun will get you back in shape as quickly as anything.

Once you have the chops mostly back in shape, find a community band, orchestra, brass quintet, whatever, and play with them. The social aspect of playing is very important in getting back in the swing of things.

If you are close to a major city, find someone you really respect and get him/her to listen to you. Notice I didn't say take lessons?

It sounds like you just need a jump start. Find the WHY of your playing or go do something else. That is a bit harsh, but the decision is yours ultimately.
