I've had one of these--you could smell the smoke before it came out of the case! In my situation, a good bath for the horn in warm water and dishwashing detergent, and snaking out every bit of tubing, scrubbing the valves, etc. followed by a thorough rinsing did the job. The case, still stank however, but a good airing out fixed the problem fairly well, though there's still a faint odor of old stogies present, so my horn resides in a gig bag.

At worst, you'd need a chem flush (not a bad idea anyway). There's a product that's been advertised on TV called Febreeze, I think, that claims to remove just about any odor from fabric for the case problem. You can do both jobs yourself if you need to.
